TERMS AND CONDITIONS OF USE FOR MyChartPLUS


MyChartPLUS is an online tool. It is a safe way to see parts of your Hartford HealthCare ("HHC") electronic medical record. You, or anyone you authorize (a "representative"), can use it. Information in MyChartPLUS includes prescription information, basic test results, summary medical history, appointments, and similar information.

To use MyChartPLUS, you must agree to these Terms of Use. HHC has the right to change these Terms of Use at any time.


DO NOT USE MyChartPLUS IN AN EMERGENCY. In case of an urgent or life threatening health condition, go to the closest emergency room or dial 911.

General

  • You must update your email address on file with HHC when needed.
  • You agree HHC can email you to the address you give HHC. For example, HHC may send you emails that will tell you of new messages in your MyChartPLUS Inbox.
  • You agree to read the messages in your MyChartPLUS Inbox.
  • You agree to answer these messages in an appropriate time and manner. Messages you send via MyChartPLUS may be sent to the appropriate department for handling.
  • You agree that HHC staff, besides your healthcare provider, may access these messages.
  • You agree that HHC staff may access your file as needed to deal with your request or support the operation of MyChartPLUS.
  • You cannot use MyChartPLUS for fraudulent, harassing, offensive, threatening or illegal activity. Unauthorized use of MyChartPLUS may result in a claim for damages and/or be a criminal offense.
  • If HHC suspects you are using MyChartPLUS for fraudulent or inappropriate activity HHC has the right, and is the only one who can decide, to block access to your account.
  • HHC can also block your account if it suspects these types of activities are connected with your account.
  • HHC can block your account without notifying you ahead of time.
  • HHC has the right to investigate and notify law enforcement.
  • You may not create any links to MyChartPLUS.
  • You can stop using MyChartPLUS at any time.
  • You can close your account at any time. Contact your provider or the Director of Health Information Management, Hartford Healthcare to close your account.
  • A “proxy” is a person who represents a patient. If you are a representative (proxy), you agree to access MyChartPLUS for the patient only as proxy.
  • A proxy agrees to access the account for the benefit of the patient.
  • If you are a proxy for a minor, HHC will end access to MyChartPLUS for the proxy and for the patient when the minor turns 13.
  • If for any reason you are no longer a proxy, you agree to immediately stop accessing MyChartPLUS. You also agree to contact your provider or the Director of Health Information Management, Hartford Healthcare, to cancel your proxy access. The patient may still access the account, or name a replacement proxy.
  • You may grant temporary, one-time access to your medical record to any of your healthcare providers via the Share Everywhere feature. The provider will be able to see all of the information available to you in your MyChartPLUS account and add notes to your medical record. If for any reason such access has not terminated in a timely manner or you otherwise would like to terminate such access then you may contact the Director of Health Information Management, Hartford Healthcare, to cancel such provider access.
  • For now, access to HHC is free. But HHC has the right to charge for MyChartPLUS at any time in the future. If HHC charges for access to MyChartPLUS, you can use it if you agree to updated terms of use and pay.

Correctness and Integrity of Information

  • HHC tries to guarantee the correctness of the information in MyChartPLUS. But it is possible that is has typing errors, incorrect information or other errors.
  • If you think the information you find on MyChartPLUS is not correct, call your provider or the Director of Health Information Management, Hartford Healthcare.

Reports/Documents in MyChartPLUS

MyChartPLUS gives access to some documents. For example, reports, laboratory results, and discharge documents. These documents may need additional review and interpretation from your provider(s). MyChartPLUS may show these documents before your provider has the chance to discuss their content with you. If you have questions about such documents, call your treating physician.

Safety/Privacy

  • Do not share your unique identification or password with anybody.
  • Do not give access to your MyChartPLUS account to anybody.
  • You are responsible of changing your password when you think someone is accessing your MyChartPLUS account without your permission.
  • When you use a public computer, it is very important to take safety measures so the next user cannot access your information.
  • You are also responsible of keeping the computer you use to access MyChartPLUS safe.
  • When you share, lose or give your password; when a non-HHC computer is not safe enough; or because other actions or omissions that are out of the control of HHC result in a violation of the privacy of your medical information, HHC is not responsible for damages and also denies all damages or accountability of it.

Availability

    HHC will try to keep MyChartPLUS available and convenient to our patients. But sometimes MyChartPLUS will not be available because:

  • HHC runs normal system maintenance
  • HHC runs normal system back-ups
  • HHC may sometimes have computer, telecommunications or other failures
  • You have access to MyChartPLUS only when "available". HHC will not be accountable when there is no access MyChartPLUS.

Third Party Sites & Applications

  • MyChartPLUS may contain links to websites run by third parties. MyChartPLUS also allows you to connect to a MyChart account that you may have set-up at another healthcare organization. The links are there for your convenience only. You may also access your MyChartPLUS account and the information within your account via a third-party application (app) of your choosing.These apps were not created by or for Hartford HealthCare or any of its affiliates (HHC). The apps are provided solely as a convenience to you. Approving requests for access allows third parties (the app, its developers, and others) access to your personal health information. Once you authorize an app to access or transmit your personal health information, it will not be protected by HHC privacy and security policies and is subject to the Terms and Conditions and Privacy Policy of the app. The app may use the information you provide for advertising, data collection or sale, mailing lists or other purposes. Review the app’s Terms and Conditions and Privacy Policy to be sure that you are comfortable with the app’s use of your data before granting access. Apps are not under HHC’s control and allowing access to apps is not an endorsem*nt by HHC of the app. In no event will HHC be liable, directly or indirectly, to anyone for any loss or damage resulting from or relating to any use of the app.
